Club Volunteer of the Year - Phil Cousins Award

Applications have now closed.  Good luck to all the nominees.

The Award
The Award was introduced by the Club in 2016 and is named in honour of Phil Cousins, a Life Member and long standing past committee member who has been volunteering for the Carine Junior Football Club for several decades.

Given to a Club volunteer who has made a significant contribution to the Club or an individual team over the past season, the Phil Cousins Award is an ideal way to recognise those that go above and beyond to keep our Club moving forward.  Contributions made over previous seasons should be noted in the application and will be considered.

Past recipients
Jeremy Jury, Inaugural Girls Football Coach (2017)
Belinda Greenhalgh, Auskick Coordinator (2016)

Eligibility
Any current Carine Junior Football Club team official or contributor to the Club in a recognised capacity is eligible to be nominated for this Award.

Nominations
Nominations are now open and will close at 5.00pm on Friday 31 August, 2018. Nominations are to be made via the form below and must be 300 words or less.  Should further background be required the review panel will request this directly from the proposer.  The number of nominations per team is limited to two.  Nominations should include the full name of the proposer (including a contact number) and the nominee as well as the relevant team name.

Review Panel
A panel has been selected to review the applications and should they require any additional information on the candidate they will contact the proposer directly.  Their decision will be ratified by the committee and will be final.

Award Presentation
The Award will be presented at the annual Open Rules Presentation Night on Sunday 9 September 2018 at the Warwick Hockey Centre.  All nominees will be invited to attend.
